Student to check clearance in Animo.Sys

The shifting application and shifting payment (which is considered non-refundable) will be deemed CANCELED WITHOUT REFUND if hold status was incurred during the processing of the shifting application
2. Access online form

Student to access and fill out the applicable online Google form of his/her current College

Student to seek permission from his/her Parent/Guardian on Record (PGOR) regarding application for shifting

Student to attach a valid ID of (PGOR) on the Google Form.
3. A. Evaluation of the Sending or Current College APOs

The student’s current College APOs shall evaluate the application for shifting
4. Endorsement of the Sending or Current College/Academic Department
-
The student's application for shifting shall be forwarded to current College Associate Dean/Academic Department for endorsement
5. Payment of Shifting Fee (Php 250.00)

Once application has been evaluated and cleared by the College/Department, ESH to email the student the following
Payment Instructions
- Assessment for the shifting fee payment
- A google link where the students will upload a copy of the receipt once paid
- Shifting fee shall be Php 250.00 Student may pay at the bank (Metrobank, Unionbank, UPCB) via over the counter bills payment slip (BPS)

iii. Reference Number: Student ID Number
Students have four (4) working days after release of assessment to pay and upload the proof of payment
Shifting Application fee is considered non-refundable
6. Evaluation of Accepting College/Academic Department

Shifting applicants/students shall be informed via email that their applications shall be forwarded to the Accepting Department Chair/AD for evaluation and decision.
7. Approval from Accepting Associate Dean/Department Chair
8. Shifting applicants/students shall be informed via email once their application has been approved
9. Check Degree Program in MLS Account

The decree code of the approved program must be reflected, otherwise please send email to registrar@dlsu.edu.ph

Enroll courses for the approved program during the online enrollment period
